COMMUNITY SUPPORT COORDINATOR: This full time position provides a full array of services to consumers in our Casemanagement program. This includes assessment, treatment planning, linking consumers to appropriate services, advocating on their behalf, providing support, client-centered consultations, doing community based outreach, monitoring effectiveness of treatment, and providing crisis services as needed. Bachelors degree in relevant social service field is required. All Heritage positions require a valid driver's license, proof of automobile insurance, and reliable means of transportation.
